MEDICAL FORMS
A completed health history form is due at the time of registration. No child is permitted to
attend camp without a completed and signed form on file for the current year. Immunization dates are required. A physical is NOT required. A physician’s signature is NOT required. Summer 2011 campers may update and approve the information previously provided. If your child requires medication or needs to carry an EpiPen at camp, note it on the above form. In addition, a Medical Distribution Form signed by a physician needs to be returned to the Y prior to the start of camp. This form is available online or at the Welcome Center.
